Are you using NWN's menu system to launch? If so, try to find NWN.exe in the installation directory (I installed to G:/NWN/). Make a shortcut on your start menu and assign it administrative rights.

That's what it took for me and it worked fine.
I got the Toolset working without any problems using the menu system.

Tried that as well. Still didn't want to launch. Trust me I ran the full gauntlet of fixes on a few threads on the NWN forum here and on bioware's site, nothing worked. After a long and frustrating weekend I finally just gave up.

I think I ended up reinstalling Painkiller and working out my anger and frustration shooting things for a while. But, thanks for the suggestions. Both of you.

Just to let you know, my copy of P.B. Winterbottom (which I got for 39 pence) works fine, if a little slow given my PC's specs.

Does V-sync not cure it? If it's not available in the game itself and you have an nvidia card, if you go to the Nvidia Control Panel (in Control Panel) then goto the Manage 3D settings tab & then the sub-tab that says "Program Settings" you can select Sacrifice and then find it on the features list & force it on, you may also want to try it in combination with & without triple buffering. (If it's not on the list you can click add and manually add the .exe file)

Sorry if you've tried that already or if you have a Radeon Card (haven't owned one of those for ages.)

Yeap.
Tried that.
Also tried the compatibility assistant.
Tried reinstalling drivers, using the nvidia inspector as per the thread instructions in the sacrifice game forum.
None of them worked.
