Humongous installs because the game just has to have a separate copy of each movie for each supported language.
Long quick time event sequences.
Non-scalable system requirements and non-optimized code that runs like crap despite not looking particularly good.
Helpful "Press <gamepad button> to <action>" messages when you're using Mouse + Keyboard controls.
Bad voice acting.

Generic Fantasy settings. Stick a fork in 'em, they're done for me. I'd prefer hybridized settings like Arcanum's Victorian-Fantasy steampunk setting, or historically-based fantasy settings like Lionheart: Legacy of the Crusader.
Every. Word. Of. Dialog. Spoken. I hate this trend because all conversations tend to be short and shallow, and if you're replaying the game, tedious to slog through. Dialog can never be like it was in Fallout / BG / PS:T because now developers stipulate every goddamn thing must be voiced. Sometimes there are exceptions that work, however. Vampire: Bloodlines comes to mind, I think because the rest of the game's setting, characters, and plot is so enthralling.
Less of anything WoW-influenced, whether in style or gameplay. Including MMORPGs.
Sandbox RPG games that are deadly dull in their open-ended, homogeneous NPCs, locales, and general lack of focus.
Others have already listed unskippable cinematics. There's probably a slew of stuff I would want less of in games but those above most come to mind. However since I very rarely purchase new games anymore I can't comment on newest trends.

Weapon and armor "customization" that allows you to make a small amount of crap that is never worthwhile after you get a few quest based items.
Lack of customization for towns in RPGs without installing a mod.
Bland, short FPS single player campaigns.
Survival horror, that requires neither survival based thinking or horror.

I wouldn't play video games if I was bothered by surreal, over the top stuff. Hyper-sexualizing both sexes has been with us since even before video games. So whether it's Duke Nukem or Lara Croft I don't much care. Personally I'd say if you're doing it, do it well, but that applies to the whole video game, really.

Bugs. The game should be tested properly, before putting the game on shelves.
Generic characters. Generic villains, generic heroes, etc.
Bad controls. The game sucks if you have difficulty to do a simple jump...

Unrealistically polarised moral choices: "Hang on sir, I'll render judgement in a few moments I just have to decide whether to wear my glowing halo or my evil twirly moustache". Also don't let us fix everything with money, every decision in Fable 3 was fixable by dumping a quarter million into the treasury and if you bought businesses early, you were swimming in money
Quicktime Events: Not only are they a lazy excuse for not making awesome moves actually playable but extended ones are frustratingly hard for an uncoordinated spaz like me. God Of War 3's Music Box Hero, Fable 3's jobs, Red Dead Redemptions Five Finger Filet... all nearly made me snap my controller and its not like they couldn't have made those things actually playable
Shocking twists where you find out that your best friend and mentor is really the big bad guy who was the model of innocence for as long as you've known him and was really only using you for a selfish purpose: 'Nuff said

A good list of crap.
I would especially like to repeat what was said about the anime style. I really dislike it. All those stupid haircuts, too large eyes and ridiculously over-sized weapons. There is too much anime and world of warcraft influences in the art direction in many games today.
Fantasy and science fiction games need more realism not that crap. Just because it is fantasy does not mean that it has to be stupid. And if you are going for the dwarfs and elves, do it good or do something else. Why not do something else?
TheCheese33: Less male privates. Yes, I know GTA IV's biker DLC was the only mainstream game to have it, but one is too much. It's not funny when they flash you in movies, and it's not funny when you see pixelated junk in games. Why this negativity about male nudity? Is there something about it that scare you?
In many pre-christian and non-christian cultures like the romans and indians the male phallus was revered and worshipped, not looked upon with fear and disgust.
Why do you see so much of the penis in porn movies? Do you think there would be much of it if heterosexual males did not enjoy the sight of it?
As for female nudity I wish there were more of it where it was needed and l wish that there were less sexualized females where it was not needed. We all know that the last will not happen and that the first will though.
