I notice some of the games I just bought when I downloaded the game they also have a patch file. Is this already incorporated into the game install I just downloaded or do I have to run the patch after the install?

Currently noticed it for the Gothic Series and Divine Divinity Series.

No need. The installers are always fully-patched. The patches are for those who have a previous version already installed and don't feel like reinstalling the entire thing.
As you can see in this screenshot, you just need to check whether or not the installer version matches the patch version. If they match, you don't need to download that patch.

Nope. GOG incorporates the most current patch.

You shouldn't need to run the patch unless it was added since you last downloaded the game.
It should be included in the installer.
It is only listed separately for those who don't want to re-download the whole game...
Check the version numbers of the patch and the main installer. If the patch's version number is higher, install it.
SCPM is wise.
