I don't remember there being any ladders to climb in San Andreas. I always has to take stairs or climb up boxes or other solid objects.
Dual wielding is a bit of a mixed bag in the game. I remember dual wielding the sawed off shotgun and shooting all for rounds into a passing car, there was a very satisfying BOOM-BOOM-BOOM-BOOM and the car spun around almost 180 degrees from the impact. On the other hand it takes a lot longer to the reload two weapons.
Don't forget to do plenty of exploring around the city. The vehicles all have skill sets that you can level up and this will help in some of the missions if the game thinks you know how to drive.
