For sheer gore, it's hard to match the silliness of Dragon Age: Origins, although that's only hack and slash-ish on the consoles.
I'll second (or third, or whatever vote we're on now) the awesomeness of Shadow of the Colossus, even though it's not remotely what you're looking for.
I enjoyed Revenant, although the game has its quirks. It's got great combat and graphics that were impressive for the day. The magic system is probably best left undiscussed, but it's not bad. Just singularly not memorable.
Nox, is, well, somewhat underwhelming. If you really liked Diablo, you'll probably like this, as it's very similar with a bit more of a roleplaying twist. it is, let's say, a workman like effort. Everything works exactly as it should, and the gameplay is perfectly solid. It just doesn't seem to thrill me. Who knows, maybe you'd like it better than I.
To suggest a non-PC game (gasp! shriek!), Castle Crashers is a bloody, hacky, killy, stabby romp, available on 360 and PS3. The story's ending is a bit, uh, nonsensical, but the gameplay's definitely there.
Since I'm GOG's soulless marketing flack, I should take a moment to point out that Die By the Sword is a gloriously gory action RPG...thingie, and is currently available on GOG at this very instant. It's the only game I know of where the combat mechanic involves dismemberment via sweeping mouse strokes. Leaving a pile of body parts behind you as you go on killing sprees feels very...Conan. The muscle-bound warrior, not the red-haired comedian. I'd call this a solid recommendation, with the caveat that I played it back when it was a new release, and don't know if it does anything quirky on a modern machine. Pro tip with this game: a trackball makes it much easier to play.
Finally, and probably my strongest recommendation, there's Darkstone. It's more or less Diablo, turned up to 11. Control two characters simultaneously (this can get confusing), fight more dungeons with more bosses and more loot against more...somethings. Look, it's a complete and blatant rip off of Diablo, but in the interests of not starting a fistfight, let's call it an homage where they took everything further. It's a blast.
