For the digital content, such as games, etc., Amazon only looks at the address you set to default. If it's not a US address, it won't let you purchase that content.
They check your IP. My wife bought Dollhouse on Amazon Instant and warched an episode; once she came to visit me in Warsaw, they blocked the content. :(

bansama: Then creating a brand new account while using a US VPN should work. As long as you're using an IP address that isn't flagged as a known VPN.
The account was made in the US, oh five or more years ago and, with that singular exception, is only used for US transactions. It's not like they don't know where the account is based in, they just elected to not let me watch my content from a different country.

I was not amused.
