I get this from time to time, it's actually my monitor/TV. I switch channels or turn it off/on again and it fixes itself.
If not, might try for a different resolution in the DosBox settings, since lower resolutions aren't really supported much anymore instead they just up-resolution and do simple scaling. So 640x480 just doubles pixels with 1280x960, etc.

So full screen isn't going to work for you. Gotcha. I know some monitors don't want to work..
