Grim Dawn is to my knowledge handcrafted - not randomly generated - at least not that I'm aware of - maybe there's some randomisation - as found on reddit
there's no completly randomized map, but different locations of stuff. This counts for secret dungeons, elite mobs and for ch'thonian rifts (your spawn location in those is randomized, too. And I think the shop keeper in the rogue like dungeons is not set, either.
As for class combinations... there are 9 masteries - of which you can choose up to 2 so 36 combinations with the two expansions - http://www.grimdawn.com/guide/character/masteries.php
