Just to add to @timppus good information, when you run the installer there is an options button, click it and it will show you where the game is installing to. Make sure you do not install to (or in fact anything ever) to program files or program files x86. I create a folder called c:/mygames, and install into folders on there.
I'm betting on SmartScreen/antivirus interference here.

Your profile mentions 3 games and 1 hour played, so I assume you are using Galaxy in some capacity.

Even Galaxy works just fine with no internet. So that's likely not even the issue. Galaxy DOES NOT REQUIRE an online connection to play games it has detected.

EDIT: Installed KoTOR and disabled my internet and it still works fine through Galaxy on a Win 10 computer.

Yeah I presumed that should be the case, but then I recall the same was said about Steam (ie, you can always play your single-player Steam games in the offline mode without internet), yet that offline mode failed me just when I needed it, ie. Steam wouldn't let me go to the offline mode when my internet was down, apparently it was something about it insisting downloading some update which it couldn't because of no internet, and I couldn't use it until it could connect to internets once again...).

So just to make sure it wasn't any that kind of "Well, it SHOULD work..." case, especially as I can't test it myself (no Galaxy installed).

I get it. I just tested it myself since you could not, and decided to quote you so you'd have the information. I don't deny that there may be some games that are affected in some adverse way, but KotOR is not one of them.

EDIT: As a professional who troubleshoots problems all the time, step 1 is to eliminate any variables possible to figure out if they cause the problem, then once all are gone, reintroduce them one at a time until you find the problem. So it's sound TS advice to do that as well. I wasn't mindlessly raging or anything, just trying to help.

1. Close Galaxy. Don't use it. Heck, uninstall it for all I care.

2. Take your favorite web browser, e.g. Chrome, Firefox or EDGE (just joking there, EDGE is never a favorite browser), and head to the web site https://www.gog.com/

3. Click on the sign-in link and use your GOG email address and password to log in.

4. If you have two-factor authentication enabled (like you should), it may be it asks you to provide an authentication code from your email. Do that if needed.

5. Hover the mouse over your account name in the upper banner and select "Games" from the menu that appears.

6. Click on any game on the list, scroll down the page, and click on the "DOWNLOAD OFFLINE BACKUP GAME INSTALLERS".

7. Download all the files that are linked below that part, one by one. If it is a small game, there may be just one .exe file. If it is a bigger game, there may be one .exe file and one or more .bin files. There may be also several patch/update files and DLC files; patch/update files are normally not needed as the main installer is usually supposed to be up to date (the patches are meant for people who had an older installation of the game), but check whether the main installer is the same version as the update(s).

Extras don't have to be downloaded, they are not needed to play the game.

8. When you have downloaded all the installers to the same directory (by default your "Downloads" folder in Windows), click on the .exe file (Type = "Application") and the game installation starts.

9. Optional: if you downloaded any DLC installers or the game needs patches, install them now, similarly as you installed the main game.

10. After the game installation is finished, don't click the "Launch" icon in the installation screen, but exit the installation post-screen and start the game from your Windows Start menu, or the desktop icon if you told the installer to create a desktop icon. (I say this because some have reported clicking on that Launch button tries to start the Galaxy client, if it is installed). This should not depend on your internet connection so if the game doesn't start, it is probably some other problem.

In theory, running the Galaxy-installed game with the .exe file should work, but since I don't use Galaxy and have no way checking if that works, I am not suggesting that. Generally, if you wish to play your GOG games offline without an internet connection, it is a better idea to download the game offline installers and install the games with them. With those installers, you can install the game on another PC than the one where you downloaded the game, e.g. transfer the game installer files to your offline PC (without an internet connection) with an USB memory stick or similar.

Total freedom; DRM-freeeeeedom!
It looks like it's worked so far for one game. I went ahead and deleted/uninstalled EVERYTHING (clean slate and all that), and created a new separate game folder as suggested by nightcraw1er.488. Took the laptop offline when it finished installing and launched the game from the folder (had to run it as admin the first time) and it worked. Hopefully it will keep working. I'm doing this one game at a time. Shouldn't take too long, I only have three.

Yes, I recommend avoiding windows folders as windows has UAC, a control on those files so “bad things” doesn’t happen. Unfortunately that tends to interfere in most normal operations, so I avoid those folders completely and make my own.

So are you now installing the games with the offline installers (without using Galaxy), or with the Galaxy? Paladin confirmed that the games should work offline installed with Galaxy as well (at least if you run the executable directly), but I guess using the offline installers is the sure way.

I don't know where Galaxy installs the games by default, but the offline installers install the games by default under C:\GOG Games\, so as long as you have enough free space in the C:-drive, the default setting with the offline installers should be fine. They do not install under "C:\Program Files\" or "C:\Program Files (x86)\", unless you specifically tell them to.

I don't know how Galaxy acts by default, ie. does it install itself by default under Program Files, and then the games also within the same directory, unless you change it? And whether that might cause problems for you?

Can you please clarify what you mean by "launched the game from the folder"? If you installed a game with the offline installer, I don't think there is any reason to run the executable directly from the folder into which you installed the game. Then again I am not sure if by "folder" you just mean the Start Menu folder in which the shortcuts to the GOG game are.

With many games, running the executable directly can be a bit complicated anyway, e.g. DOSBox games. For instance, with Dungeon Keeper Gold (which is a game using DOSBox) the correct way to launch the game is:

C:\GOG Games\Dungeon Keeper Gold\DOSBOX\DOSBox.exe" -conf "..\dosboxDK.conf" -conf "..\dosboxDK_single.conf" -noconsole -c exit

So you need to run DOSBox.exe with lots of extra configuration options. The shortcut that the game created upon installation created that needed shortcut with correct options into the Start Menu, and also the desktop if you told it to.
