It was a 3d real time rpg with usual sword and magic setting. The unique feature is you start out leaving a castle lower level going into a cave to find your sister at the request of your father (he may be the king). Your sister ran away because she was being forced into a marriage. There is a weird relationship between you and your sister and as you progress you get powers from her.

I would have played it within the last decade, so the game is not too old or very new. Strangely I have a fairly vivid memory of playing it, but it matches nothing on my games list. I hope this isn't some kind of senior moment. Any suggestions would be appreciated.

I'm pretty sure it's "The Dark Eye: Demonicon", from 2013.
https://store.steampowered.com/app/215630/Demonicon/

(Translated) quote from a German review:
"In order not to reveal too much of the story of Demonicon, we will only outline the basics:
We slip into the ragged clothes of the run-down anti-hero Cairon, who is searching his sister Calandra.
Calandra has fled a forced marriage, and hides inside an old temple mountain.
Admittedly, there she is save from her pushy groom, but not from hordes of equally pushy monsters."

(Translated) quote from the German wiki:
"However, "Father" stole two babies at the ceremony, one of which was his own son, Cairon, and the second baby was a daughter of Seghal, Calandra.
"Father" raised the two children and did everything in his power to hide them from Azaril.
He also tried to prevent their gift from awakening.
This happens either when the blood of two awakened persons mixes or when they have sexual intercourse with each other.
Both children grew up without knowledge of their origin or possession of the gift.
[...]
Calandra notices at night, that time and again people are taken by someone to the Molchenberg.
Calandra decides to follow them and free the abducted people. She does not tell her father or Cairon, however.
When they notice that she has disappeared and they get the hint that she has descended into the Molchenberg, they fear that she wants to avoid marrying Falk.
"Father", who recently lost a leg, cannot follow her, so Cairon descends alone into the Molchenberg.
He finds Calandra, but both are injured in a fight.
They bandage their wounds to each other, which father has always strictly forbidden, and their blood mixes.
The gift awakens and from now on both can do magic."

Sounds exactly like your game, wouldn't you say?
Thank you very much. That does sound like it. I like the Dark Eye games, but I'm not sure where I ran across this one. Time to start digging.
