I'm looking for a very specific game genre..
I'll maybe starts by listing the different turn-based games I have played, and what I don't like in them :

Fallout 1 & 2 : the first turn based games I have played. While the combat system was really great, I didn't like the fact that you had to make a choice between increasing RPG-related or combat related skills. Otherwise, it was a really good game ; with a bit more of loot randomization it would have been almost perfect to me.

Arcanum : One of the greatest game I have played. Charism apart, the specialization points were only combat oriented - as far as i remember -. The only con to me was the lack of randomization. If you played a story twice, you could remember where the good items are ; I prefer random loot because of this.

Dungeons of Dredmor : Too many stuff to figure out. You get items you have no idea what are their use. I'm not a fan of this. Also, I prefer when you have several action points rather than when it's one move = 1 turn.

King's Bounty The Legend : While this game could have been perfect to me, the major issue was that you fought with monsters, not with your hero.. You had to take care to always have a large enough army, and I didn't really like this aspect of the game.

Heroes of Might and Magic : well, it's supposed to be the fusion between King's Bounty and another game - I can't remember the name -, so yeah it has inherited the cons of King's Bounty :p

Final Fantasy Tactics : while this game is really fun to play, it's unfortunately not on PC :/ And I would have liked more complexity on the items and skills.

Dofus : if it wasn't for the monthly subscription, and the farming aspect of the game, this could have been a perfect game to play for me

Fallout Tactics : This game is really fun. Mostly combat oriented. Just too bad that there is no loot/encounter randomization.

So yeah, I guess I'm looking for a kind of mix between hack'n'slash for the randomization, and FFT-like turn and party based RPG. For example, FFT with 'Diablo 2'-like items and skills.

Do you know any game to your knowledge that does that ?

Also, I would preferably play one who is not F2P, and not on a monthly basis.
A game that you pay once, and then you have access to the entire content.

Sounds like a roguelike would be what you want, seeing as you've got the randomization of gear drops and levels, turn-based combat (well everyone moves/attacks at the same time) and it's just dungeon hacking. You've also got merchants in later parts of the game. Great variety in race/class combinations too.

I'd recommend NetHack with the Vulture tileset (which gives the game isometric graphics similar to Diablo or Fallout):

I say Temple of Elemental Evil. It's Fallout/Arcanum-ish (even Baldur-ish, one would say), but has the best battles and battle interfaces I know (okay, second to Fallout, I really liked the Fallout fights).

Expeditions: Conquistador has combat system quite similar to King's Bounty and while you don't fight with your hero alone your army is more like party with developement and individual characters.

Not much of loot, IIRC, though.

I think Wakfu is similar to Dofus, and it doesn't have the monthly subscription, though it has a real-money shop for certain in-game items.

You have the choice in Divine Divinity to play it as an action rpg like Diablo or more like fallout (turnbased). Never tried it turnbased so I dunno if it's good like that though.

I never knew that.
I thought Original Sin was the first Divinity with turn-based combat.
I can't recall if the Geneforge games had randomized loot or not, but otherwise, those and anything else by Spiderweb Software are worth checking out.
Based on your criticism of King's Bounty and HoMM, I would like to strongly recommend Disciple 2. Unlike HoMM and King's Bounty it's not a 'numbers game' and you do fight with your hero. HoMM3 I believe is considered by many as the titan of fantasy TBS games, but Disciples 2 overtakes it in several regards. Disciples 2, for me, has a far more intoxicating atmosphere, a cooler story (well, there isn't much of a story in terms of content, but unlike HoMM3 I didn't find it cheesy/uninspiring in any way), a more believable world, and impossibly cooler and more badass creatures and characters that are all incredibly well designed and highly detailed (each battle is a feast for the eyes). Make no mistake, I love HoMM3 as well, but it's definitely in 2nd place behind Disciples 2 in my book.
As for difficulty, HoMM3 is definitely the more difficult one, by quite a long shot, but that is only because HoMM3 is incredibly difficult a lot of the time. By comparison Disciples 2 is merely difficult most of the time, and only occasionally of moderate or high difficulty.

Don't mind the game page too much though. For some reason gog has very old and low quality screenshots on it.
The Might and Magic (not heroes) games have random treasure drops.

Wizardry-likes (as I call them) also have random drops. This includes Wizardry 1-5 (not 4) (not available on this site), Bard's Tale 1-3 (emulated versions included with the Bard's Tale game from the last decade), and, of course, Elminage Gothic.

I could also mention Lords of Xulima, but that game has the same issue of having to choose between combat and non-combat skills.
Adding to what dtgreene adviced, also Agarest (seeing as you mentioned FFT like games). Though you don't usually get loot in Agarest in its pure form, you can craft gear from whatever you get in battles - it is nearly a science on its own. You can further customize gear with various attachments, salvage old gear (or rather - convert) into various components, and so on.

I wouldn't call what you seek as hack-and slash though (hack-and-slash is usually synonymous for Action RPGs). I'd rather call it turn-based RPGs with elaborated loot systems.
I'd say original sin is probably your best bet.

I would also recommend blackguards, although there is no random loot there (at least in the 1st one).
Hmm. Difficult requirement. I guess there is still no game that really suited your requirement. So.. something that has randomized loot but with turn based tactical battle system ? I ever played a flash RPG on Kongregate called Crystal Story. Well, the battle system is typical turn based jRPG. But the weapon drop by the enemies is something similar like loot system where various modifier applied to the weapons. But I don't know if that kind of "small game" is suited to your taste.

I think rogue-like type of game is more suited to your taste. Perhaps... Tales of Maj'eyal ? It is kind of turn-bsed hack and slash and also has loot system I think. Also available for free on their website if you want to try it first (full game but without bonus features). Also available here on GOG.
I think Wasteland 2 is a good fit for you. It's similar to Fallout Tactics, with a bit more RPG...
