Or when the lack of a comma is important:

Which is a real thing:

dtgreene: I just thought of a rather famous non-game example.

There was one print of the King James Bible that omitted the word "not" at a rather important spot. In particular, in that print of the bible, which has been named the "Wicked Bible", one of the commandments read "Thou shalt commit adultery.".

Remember, the word "not" is important; make sure you remember to use it when necessary.
Of Course missing the entire front page has also been somewhat problematic

This is a work of fiction. Names, characters, businesses, places, events and incidents are either the products of the author’s imagination or used in a fictitious manner. Any resemblance to actual persons, living or dead, or actual events is purely coincidental.
Thought of another one while writing a post in another topic.

The editor for the original Dungeon Siege was atrocious. Entire menu options, many of which look like they would be useful, don't work, and many of them will crash the editor. It really should not have been released in this state.
Not sure if this is ‘major’ enough, but here it goes:

I played Owl Boy recently and broke the game in the first ten minutes because I misplaced a key item needed to solve a puzzle and put it in an unreachable place (I threw a rock at the wrong spot). Normally, leaving and reentering an area would reset a puzzle and the required items, but apparently not in Owl Boy (not in the first dungeon anyway). Forums confirmed that this was a straight up oversight. That was like a year after it’s initial release too.

I know it’s hard to account for all player choices, but that did seem a bit ridiculous to me.

Edit: I should mention that the only way to fix that problem would have been to start a new game. All because I threw a rock in the wrong direction.

On the backside of the Danish VHS-cover for RINGU (1998), the twist ending is casually explained...

One of the tracks on the soundtrack for Star Wars: The Phantom Menace was called "Qui-Gon Jinn's funeral".... the soundtrack was released before the film...
I regret to inform you that, at tonight's rehearsal, more bugs were found in that John Williams piece, including one wrong note in a rather tricky passage. I still wonder how that piece got released; if they had had an orchestra sight read the piece before releasing it, at the very least they would have caught the mistake in the trumpet section. It's gotten to the point where, if in doubt, we can't trust the printed music to be correct.

(Some mistakes were found in another piece as well, but but none that would be nearly as obvious, so I can see how they slipped through.)
Some mistakes are so epic that they become classics. Like how they forgot the copyright in Night of the Living Dead. Or that Mummy trailer where they forgot most of the audio.

Only if you pay very close attention, and it doesn't really make much sense until after you have seen the film. It is more like the rantings of a lunatic. But when you watch it again and know then you go "aha" (at least I did.... but I can be a bit thick sometimes). And the nature of the city itself is not reveled, I think.

edit - in a way, if you pay attention and know about Shintoism, the whole narrative and the main point in Spirited Away is kind of hidden in the first 10 minutes in the film. But that's more expert storytelling, art and directing.

I can't on my current PC, but I have heard it's a massive joke.

Yes. It is. It just stops dead in it's tracks. The plot, laid out in the dialogues before, is at most one third progressed and then the game is over without any resolution. The devs ran out of money and just published what they had at that point - and what they had is VERY incomplete.

OldFatGuy: Also, every game by every publisher since at least the year 2000. Games are released now so that consumers are basically used as beta testers to work out the final problems. It's a great set up if you think about it. Not only do they no longer have to pay for extensive testing... others pay them for the "privilege" of doing it!!! Can't say that I blame them, it really is pretty awesome.

GR00T: You know what? This point may be slightly exaggerated, but it's not far off the mark. How many of us now wait until months after release or even longer for the fully patched GotY edition instead of buying on day 1 now?
I do, the only exception I make are Nintendo Switch games, and I think I got Prey this past year within 2 weeks of launch.

Regardless, I do not buy pc games on day 1 anymore. Even when I had a PS4, I did not get PS4 games on day 1.
(Might and Magic 5 puzzle spoiler)

Something that's not quite as significant, but still should have been caught in testing: One particular puzzle in Might and Magic 5.

There is one point in the game where you encounter some puzzles from the Vowelless Knights; you are given the consonants from one saying, and you have to type in the vowels as your answer. Well, there is one saying where they made a spelling mistake:

"If wishes were horses, beggers [sic] would ride"

So, you get to this puzzle, you see just the consonants, and think of this saying. You then, knowing that the word is spelt "beggars", type in the answer with an a. Unfortunately, because the game's developers misspelled the word (and you had no way of knowing), what should be the correct answer is marked as incorrect.

Fortunately, the puzzle can still be solved, because the answer the game considers correct is given to you elsewhere, and you later get to see the entire saying (*with* the misspelling), but one of the testers should have noticed this mistake, especially since you do get to see the entire saying.

(By the way, this sort of mistake can come up in tests in school; if the answer key is wrong, students will unfairly lose points as a result, at least until somebody notices the mistake.)
