Achievements don't tell the whole story though. I have 100% achievements for Fallout 3 on PSN, but the world is too large to for me to care to explore every nook and cranny, or collect every unique item.

The closest thing to finishing a game 100% was Vandal Hearts on my 2nd playthrough. In one mission, the player is expected to run away since the opposing forces really strong. However, this time I managed to fight them off, thanks to having more guardsmen, who were durable enough to hold the choke points. Also, I unlocked hidden (and overpowered) upgrade for the hero.
