What sort of mischief would resulted in a ban?

What did I do this time!?

Bans have been known to happen, but they are rare. I think I can recall a total of 4 cases while I'm here, though never really asked why that was the case.
2 of the cases were people wrongly identified as spammers though.

If it helps I have made an effort to get someone booted and GOG told him what it was that he was doing would eventually get him booted but they seem to have forgotten that so who knows.

I see, Any other offense resulted in a ban? Like harrasment?

I do have to say that it is possible for bans to have been happening every day, but not to people I notice on the forums. Bans are more noticeable when they happen to active people, especially if we can't fathom a reason for their bans. If tinyE stops posting for a week for example, we will wonder what happened to him, but if ExiledUser doesn't post, few, if any, will wonder what happened to him.

GOG does not publicize their bans, and I do think they are correct in doing so, so it falls to each user to decide whether he wants to say that they were banned for reason X. Few do so.

Back in the good old days, when I was banned from the Master of Orion 3 forums over and over again, I just kept re-registering as slightly different usernames. yogsloth, htolsgoy, Yog S'loth, etc. etc.
