Some games have multiple files for download. One is the setup .exe. Oftentimes there is also a patch .exe. Does the patch get installed automatically as part of the install process or do you have to run it after the install is complete?

If you use GOG Galaxy, patches install automatically (if you choose that option in settings). If you use the offline installer patches have to be installed manually.
Patches are generally incremental, and to update to a newer version of the game. Look at the build number, if the build number on setup is less, then you need to run the patch, otherwise the setup and files are the newest version. Patches won't be held too long before you simply have to download the whole game to get the current version.
Ok thank you all. Yes, I'm using the offline installer.
Offline, the only game I've seen that didn't apply the patch is Hitman Blood Money

Not sure if this is optional or a rollback patch according to this:

Don't have Galaxy installed so not sure if it would automatically install it.
It's possible that there are more games like this
Offline installers do not automatically patch. They're called offline for a reason.
If you are downloading the installers for the first time, then you don't need to download and install all the patches too, if there are any on offer for download.

However, if a patch releases after your download, then yes, you'd have to download the patch and apply it manually.

So if I were to download for example the installers for Serious Sam: Syberian Mayhem right now (see the screenshot), you can see that the main installers are at the latest version (1.04b). So you'd need only those. If a 1.05 patch released later, then you'd need to download and run the appropriate patch file.

As for Galaxy, you can set it to auto-update.

If i understand, then we do not have to download the offered patches to have the latest version? just the game installer?
I have several games where the main exe appears fully patched.
There are also several patches available, but no main exe that is unpatched.
If there were an unpatched exe, that would effectively be an offline rollback option.
Alas there is none.

I do appreciate the patches still being available.
I would appreciate even more, from a game preservation viewpoint, if an unpatched exe was still available.

There was mtiple rollbacks so probably currently separate patch are more uptodate than main setup installer. And highly likely you wont get issues applying this patch. 50/50.

There is another game where patch always be as optional further upgrade of the game - Stalker Shadow of Chernobyl.

But there is some else games which have something available as optional setup or archive files in Bonus goodies download section.

There is a lot of DLCs games.

Patches by the way dont support DLCs.
