I bought recently the Ultimate Edition of Fallout New Vegas on GOG, but with or without mods, when I start a new game, Doc Mitchell's head turns clockwise at 360 degrees and his body doesn't show right animations. I heard and red about this ancient bug, but it seems to appear once again, what can be done to solve it ? Thanks a lot.

I'm having the same issue; never came across this in my steam version; bought on GOG so I could play the game without being locked into it by steam; I am running Tale of two wastelands which is not an trivial install, but I haven't been able to get the heads to fix. Drivers are up to date, I've cleared the .ini files, and the mods have uninstalled; any thought?

Despite selecting 'Ultra' for setting that and a plethora of other settings were set to minimum; so hopefully this helps others.
This same issue was reported and solved (for apparently the same person, Ewendoun) on the Nexus forum:

However the asker, presumably the very same Ewendoun, never bothered coming back to GOG to let anyone know this issue was now solved.
