For starters, I've manually set 'nVidia Control Panel' to use the 'GeForce GTX 1650 Mobile' graphics card with both the 'galaxy.exe' and 'falloutnv.exe', to prevent the OS from using the embedded Intel 630 chip.

The game is always launched from its "profile" within Galaxy. This calls up F:NV's "splash screen", where I've activated all expansions / GotY content (except the language pack). From where it continues into the game proper, without problems; I click on 'Continue', and start playing without issues.

After a few hours of playing -- and this time interval appears to be getting shorter, last time went from some 3-6 hours to about 30min -- the game just hangs up momentarily, the screen blacks out, and it crashes to desktop, with no particular, repeatable in-game action standing out (so far). Also, when the game crashes, the laptop loses connection to its auxiliary screen, with the latter just going to standby mode.

Galaxy then synchronises the savedata, and doesn't complain about errors.

Re-launching the game, and the splash screen will attempt to change the settings for "best quality". As it turns out, the discrete graphics card is no longer visible to the system, and the game is re-configuring to use the Intel 630.

The GeForce doesn't show up in "System Information", "Device Manager > Display adaptors", nor "(nVidia) GeForce Experience" programme (the latter says "no driver found", tries to install a driver, and fails).

Rebooting the computer from this point is usually "fine". The second screen will not have picture yet, and looking in "System information" shows that the card is still "missing". I've had a few BSoD's, with a error message pertaining to 'direct x kernel fail', as memory serves (the message is 'DX-something-something-KRNL-...-FAIL'), but I haven't seen this message for a few crashes now.

Shutting down entirely -- and I wait a couple of minutes as well, for good measure -- and re-starting seems to partially "fix it"; the card is again visible. From here, I need to reboot, and only now does the second screen come alive, extending my desktop. Launching the game from this point plays again just fine. It also re-sets its graphics options to 'ultra-high quality' (the "original" settings, that otherwise work).

I should also state that I very frequently switch to-and-fro between the game and the desktop (via Alt+Tab), so I can keep track of the game's map/exploration progress. I notice no stuttering in-game, nor temporary freezes, nor any other issue with the programme in regular use.

Running Win10 64bits, fully updated (as is both Galaxy and New Vegas), and as far as I can tell, no other programme on the computer causes this issue (but this is the most-intensive programme I've launched in this computer, despite the occasionally-absurd memory consumption of Firefox (3-5GiB)). I've played a few hours of "Bio Menace", also launched from Galaxy, and this problem never showed up.

Any ideas as to why this happens, and how to fix it?

Just a shot in the dark (or perhaps I should say "a low-% VATS shot"), but check your GPU temps. Get a program like MSI Afterburner and also if you are running high temps, manually set the fan speed higher if possible.

Also, dust your GPU fan!
