I'm trying to get the VC Captain of the Guard title, but I'm stuck early on.

So...I got the briefcase from Moore, gave it to Bishop, robbed his safe of the holodisk and map, agreed to kill Westin, talked to Westin and agreed to kill Bishop instead, successfully re-jiggered Bishop's safe and blew him up, went back to Westin and got paid. Now, I've talked to Westin again (only dialogue option I get is "oops"), Stark, Lynette, and McClure, and nothing about the holodisk comes up. I've used the map and got the raiders base on my Pip Boy, but haven't gone there yet.

Do I have to take out the raiders first? When I played ages ago, I thought I was supposed to give the holodisk to Westin first, but nothing. Really hoping I didn't screw something up here. :/

When you deliver Bishop's Holodisk to Westin he'll ask if you're a Vault City citizen, and if you are he gives you this quest. (If you have the quest to kill Westin you can do it before you leave, since there's no additional reward once you deliver his disk.) For a successful delivery you get 1500 xp, a H&K G11 and 2 Frag Grenades, which Randal in Vault City will supply.

If Lynette thinks highly enough of you when you deliver the holodisk, she'll make you Captain of the Guard, a position which has several advantages as decribed in the Vault City section. To pull this off you need CH 8 and getting Lynette's "respect counter" high enough by adressing her as First Citizen at every turn. The easiest way to do this requires Speech 75% and being a citizen (which you obviously are since Westin gave you his disk),
One obvious question would be, are you a Vault City citizen?
I'm not sure is that what wrong or is there any work around possible if you not.
