The game has been fine, totally glitch-free, until I played it today. I'm in the Boneyard and fighting some Deathclaws when suddenly everything goes black. I can hear the music still but nothing else seems to be happening. An error window is open but I can't see it because Fallout seems to be really reluctant to let any other window in front of it; ALT+Tab, ALT+Esc and CTRL+ALT+DEL don't work, so I have to press the Windows button to be able to close the game. It happened twice so far, in the same place but not while I was doing the same thing.

Then, after I'd finally got through that part without it crashing, a new bug happened. The screen went black, like before, but it hadn't crashed. Anything that moved became visible again and if I move the mouse cursor around it cleans away the black. This has happened about 4 times in just a few minutes. Going to the options screen resets it, but it's still really annoying.

I'm playing it on XP, with the unofficial 1.3 installed and the mod that lets you use the mouse wheel and some other things. I guess it might be that last thing causing the problem since it does change some graphical things, but I've had that installed since I started playing a few days ago, with no problem until now.

Have you tried running the high res mod along with the unofficial patch? It might fix the black screens.

Just install it and then in the options click the button to bring up the resolution options, set it to whatever resolution you like(including the original resolution if you wish) and set the colours to 16bit rather then 8bit.
Weren't people reporting similar black screen problems that were in some way related to DirectDraw or something? I seem to remember some forum topics that mentioned a solution involving deleting a certian .dll file.
