I am not sure it is possible to add the table to the game, but you can download the old shareware version (v1.1) and change the starting menu to start it and its setup program. It would be almost the same.

Edit:
I just tried and it works fairly well. I unpacked the shareware .zip file in a directory called "SW" and altered the menu; the resulting conf file is: dosbox_epin_single.conf .
You have to execute the shareware setup once, just select Sound Blaster Pro and keep everything to default and it will work.
