Hey guys,
If you think the default controls are a bit awkward, just enable the mouse look in-game, then find in your PC ...\GOG.com\Duke Nukem 3D\DUKE3D.cfg and open it with Notepad: scroll down the file until you see [KeyDefinitions]. Change the following lines as you wish, e.g.
Move_Forward = "W" "
Move_Backward = "S"
Strafe_Left = "A"
Strafe_Right = "D" ...etc!

I find that simply downloading eDuke32 is a better solution. It fixes a lot of other issues, as well.
Or you could just use the setup program...

I agree, if only because most of what makes EDuke32 what it is is my work. Just throw eduke32.exe and your copy of duke3d.grp from GOG into a dir and start it up--the default configuration that gets generated uses a modernized WSAD keyboard layout.
Hey, why won't it let me save it? I get in, then i press save, it asks if i want to overwrite the original, i say yes, then it say Access denied...wtf? can you help me out here?

Do you have the game installed somewhere other than the default directory?
I used SETUP.EXE to change keyboard settings. I got no problem with that.

Not eduke32.

You should be able to bind the mouse buttons to any action. "Under Controller Setup" the "Setup Mouse" option will let you change what the mouse buttons do.
