I have been running into an issue that started only recently. The first few days I got the game it ran perfectly but now very often when i transition screens(leaving or entering a new area like Gran Soren, Cassardis or any dungeon) the game crashes. All of my drivers are up to date and any software that might be causeng the issue is DS4Windows(for my PS4 controller) but that was working fine the first few days i got the game. If you need my specs I can post them otherwise thanks for any help you can offer.

Got this one as well, also happens every time on opening the map.

Your controller comment made me look to that, even though I've had one connected the entire time, and I'm playing fine with K&M.

My Logitech Rumble Gamepad F510 (Old PS2 style) has both X-Input and D-Input settings controlled by a switch on the rear.
I'm often switching between them to quickly disable the controller, as I rarely use anything but K&M.
That's normally, to stop it affecting K&M input though, not stop CTD's.

PlayStation's D-Input causes this issue,
XBox/Windows X-Input removes it.

D-Input is the culprit for me, and it's a simple fix to switch it to X-Input.

Don't know anything about PS4 controller's, but quick search gave this guide

It has a link to a tool called input mapper that might help, if you can't flick a switch like me.
