If running Vista / Win 7 -
- make sure you turn UAC off prior to installing the game.
- make sure you installed the game by right clicking the setup.exe and clicking "Run as an administrator" in the resulting dialog.
- make sure you have right clicked the game shortcut, clicked Properties, click the Compatibility tab and check "Run this program as an administrator".
- On Win 7 especially, it is best to install in a directory outside of Program Files.
Are your video drivers up to date?
Click here for ATI Click here for Nvidia On the DD start screen, check the lower right corner and make sure you are running V1.005A. If not, backup your saves, uninstall the game, re-download the patched version and reinstall. (V1.005A is the only version you can download from GOG since about Feb)

If you refuse the quest they will attack you. Make sure you are giving the correct response to accept the quest or try a different response. Save right before talking to him so you can reload to try different responses.
Identify all axes in your inventory prior to talking to Smiruk. If you have already found it, leave it ( and any other axes ) on the ground before talking to him. After getting the quest, retrieve the axe and talk to him again. That should activate the necessary dialog to hand it over. (He takes and keeps all axes in your inventory so only have the Slasher with you when you talk to him)
Did you or the zombie named Gregnar Brock kill any of the Orcs prior to talking to Smiruk?
If all else fails, this is a minor quest and can be skipped without consequence. Skipping it may be the only solution if your game saves are corrupted.
