Just noticed this recently, so wondering if it's a certain condition that triggers it. I was in a quite desperate situation and needed to cut corners and leave everything not directly related to keep my few remaining diggles alive (and later on the course to making more before they too died of old age) when they were all starving, exhausted and in terrible mood and I didn't have necessary buildings set up again yet (badly planned camp move and putting off patching things up until it became a spiral), and for the first time I saw items vanishing. Had one drop a few mushroom caps close to a fireplace (she was taking them to storage) and they just... melted away in seconds. Had another drop a grub next to kitchen (had picked it up and seemed to be left with it in inventory, gave up on transporting it there) and it also seemed to vanish. But the annoying part is that two caps of dead dwarves also vanished. At least one was also next to that same fireplace where the mushroom caps vanished, and it had been there for a while when I just saw a puff of smoke and away it went, though the panner and club of that same dwarf (died of old age) remained there. Don't even know where the other was, had been lying around for a while I guess, but again other items were still there when I finally got around to retrieve them, cap's gone. So couldn't make their tombstone, poor Ulli and Gerdi, won't even be remembered...

So what causes it to happen? Or, is it intended or some sort of bug?

So actually I just finished the game and I noticed that in game description in tech tree for CPR (one of the most hightech thing you can have - it revives dwarf using his/her cap) states that "caps dissapear in 1 day". So this is intended.

Muschroom caps are also vanishing quite quickly, thats usual and obviously intended.

I havent noticed any other item vanishing.
