I feel like I have to get this off my chest at last. Some context to begin with:
We are three avid ironman players who settled for devilutionx a while ago because it's the best way to play the original game in multiplayer, can't overstate the excellence of the project as a whole. However, we keep losing 9/10 games in the same manner. Last time in nightmare, we went downstairs to level 12, didn't move and got deluged by knights and spitters anyway right at the stairs and died. Didn't count, but I'd say 7-8 monsters at once. It's illogical because those monsters shouldn't be active in the first place, but this kind of bullshit happens time and time again.
(Something possibly related once happed in level 16. After opening Diablo's cage with the levers, he attacked us immediately. That's not how a light radius works.)

It sucks.

We noticed that two of us not only had the area around the stairs revealed on the minimap but also an, let's say, exclavic part of the map that we have not actively uncovered yet. It wasn't far from the stairs and in the direction from where those monsters came. I'm inclined to consider that not a coincidence.
Now as to the cause - I think that 1) it's obviously either a bug or desync or a combination of both? and 2) it might also be a devilutionx related problem? I don't remember that this happened as frequently and in the same characteristic in the original game.
I can imagine that the game somehow assigns the wrong coordinates to a player upon entering a level, then checks for nearby monsters and activates them before correcting the coordinates and relocating the player to there.

I hope somebody is aware of the problem and can provide some details on why exactly this crap is haunting us.
We have fixed a few issues relating to syncing mosnters from the wrong level that will be part of the next release.

But with out an exact way to replicate the issue it can be hard to say if that is it or confirm if it's fixed or not. But I'll try and keep an eye out.
Replicable almost means predictable and avoidable, so that would be a step forward, but if the fish also bites in muddy waters, I'd gladly check this one out. Can you foresee when the release will be?

Good news.
We just started a new game and got the map bug again (screenshot attached). Sometimes a picture says more than words.

The originally reported behavior sounds similar to the DSF buglist entry FAR OFF MAP SQUARES:

Occasionally dotted map squares, corresponding to walls and inaccessible
areas, will be visible on the map upon level entry - even when being
very far away.

I believe the community speculated that, as OP says, the entering player was briefly at that distant location on the new level, before being moved to their apparent entry point by the stairs. This was back in original Diablo, and may or may not have been fixed in DevilutionX by now.

the cause was most likely a typo in variable name, fixed over 2 years ago in devx

Yeah it's two different issues that are being refered to here. One was fixed the other one not yet.
