I finished normal, hardcore and grim modes, unlocked all weapons, and statistics shows quest progress is 100%. But the last perk and perky achievement are still locked. How to unlock it? Which mode and the necessary details?

Hard and annoying. Either I should abandon the last perk or I ask my sister to play this GEMBINE. She's a very good casual gamer. :D
