I've tried starting the game with multiple Windows options (7, ME, Vista, etc)
I've tried reinstalling on a different drive
Not sure what else I can try.

Running a 4090 with a 9800X3D. Should I try playing on my Steam Deck instead?

What System do you have?
Windows 10 or 11?

Does something appear or just nothing?
Do you get an Error displayed?

Windows 11

A black screen comes up for about 2 seconds then it goes immediately back to desktop. No displayed error.

Strange, it is exaclty the same error i had under Linux Mint, there i had to force the ddraw.dll and set the Desktop Resolution myself.
Open the Direct X Options (dxcfg.exe, in the Folder or via Galaxy) and try changing the Resolution to your Desktop Resolution and also try to run it in Windowed Mode in a lower Resolution.

If this fails, try to install DirectX9, maybe there is something left on your PC.

You could also try to make your own Ddraw.dll with Dgvoodoo.

Thanks for the advice, nothing has worked yet on desktop but I got it working on the Steam Deck. Only downside is I can't quit the game without shutting off the deck lol
