I did some testing and here are the numbers.

The zombie will hit you for (the number in brackets is after the load game):
11 hp (36 hp) - Still Kicking
15 hp (27 hp) - Pink On The Inside
18 hp (18 hp) - Lightly Broiled
22 hp (22 hp) - Well Done
26 hp (12 hp) - Extra Crispy

As you can see, this bug doesn't affect third and fourth difficulty setting. You get the least damage on Extra Crispy and most on Still Kicking. Extra Crispy and Well Done have additional enemies so the hardest difficulties are Still Kicking and Well Done.

Not to throw a wrench into anyone's research, but Blood has a rudimentary locational damage detection system, so taking a certain damage amount from a zombie's axe can vary according to where and how close player is to said zombie. I'm not sure to what degree this feature was implemented in blood but I'll try to do my own test to get some idea how it might affect any results.
It doesn't matter. The fact is, after you load the game, you're gonna take triple damage on what's supposed to be the easiest difficulty setting and the least damage on the "hardest" one.

Not on a real DOS computer, but I did load it up on a FreeDOS image in VirtualBox and it's doing the same thing, upping the difficulty after reloading from a save. It's not just v1.21, I tried it with the Blood 1.11 Shareware as well.
I'm having the exact same problem. I guess the only solution (for now) is to play it on a harder difficulty.

It always existed since 1.20 and people started noticing it often since OUWB that is 1.21 no clear fix exist for this as altering values will make either player or enemies too weak at the same time because they are shared so hex editing as someone tried to do is pointless. BloodGDX might have fixed at least for me. I never cared for that bug as on Well Done is nothing game breaking, only noticeable on Extra Crispy and easiest difficulties.
