So I bought the first Blood, and the first thing I chose to do was alter the controls to a more modern (read: usable) WASD format, with my intent to place fire and alt-fire on the left and right mouse buttons, respectively, and put the next/previous weapon on the scroll wheel. However, in the controls window of the main menu, I set the relevant commands to be modified and hit the mouse button... to find that not a damn thing happens. I hit something on the keyboard and it gets set to that instead.

I swear I've turned the internet inside-out looking for, say, a comprehensive list of console commands that would help me accomplish this, but have found nothing.

Help would be very much appreciated.

Did you modififed it in game menu or in setup menu? It certainly can be set through setup file.
I eventually settled on having Q for alt-fire and just living with the weapon switching issues.
There is another menu for setting up the mouse buttons in the Seup program that came with Blood. You can't set up the mouse buttons in the same menu as the keyboard buttons.
I hope you get this working man, I can vouch for sure that when I used to play this game religiously back in the late '90s I always used the mouse in the exact format that you have stated.
Unfortunately I cannot help you with a fix though :(
H'okay, I've been able to compromise. The left mouse button, on its own, decided to set itself to primary-fire.

Alt-fire is set to the Q button, but I'm not really using alt-fire that much at all. It's a fun game, though!

You can set up the buttons for the mouse through the setup program that came with the game. I have my alt fire set to my right mouse button.
Probably too late for the OP, but incase anyone else has this problem:

In the Blood folder, there's a file called blood.cfg. Open it with Notepad, and scroll down to this bit:

MouseButton0 = "Weapon_Fire"
MouseButtonClicked0 = ""
MouseButton1 = "Move_Forward"
MouseButtonClicked1 = "Open"
MouseButton2 = "Weapon_Special_Fire"
MouseButtonClicked2 = ""

They are the lines for the 3 mouse buttons. 0 = left, 1 = right, 2 = middle. To set the right mouse button to alt-fire, change the above to this:

MouseButton0 = "Weapon_Fire"
MouseButtonClicked0 = ""
MouseButton1 = "Weapon_Special_Fire"
MouseButtonClicked1 = ""
MouseButton2 = ""
MouseButtonClicked2 = ""

.. and save. Enjoy!
Hello, I already made macro for my gun in game, but if i tap 3st button for activate it, it just go way what i get in the bloody, but it dont shooting, how I set up it fire too?
