How do you control this game? Do you use the pre-maped control? Do you aim with mouse? Have you changed it to WASD configuration?
I have changed to a typical WASD configuration (with space to jump, e to use, shift to crouch and so on) and fulfill my aiming needs with my mouse (and bmouse). Despite my loving for older FPS's, I much prefer "newer" control methods and I need mouseaiming :P
I use classic WSAD with F as use, C to prone, space to jump, Q and E for look up and down to reach some switches in maps or addons and mouse is used only to fast rotate my character view so yep, no need for mouse aim in this game because it kills immersion and depth perspective of things.

Launching the setup utility. If my memory serves right, there will be a nice shortcut in your installation directory.

Blood 1 (and One Unit Whole Blood too) has in-game setup for setting the controls:
In the game menu go to OPTIONS > CONTROLS > CONFIGURE KEYS,
set the
- MOVE_FORWARD to W
- MOVE_BACKWARD to S
- STRAFE_LEFT to A
- STRAFE_RIGHT to D
- MOUSE_AIMING to any key (default U) = this key toggle the mouse aimiming in game

and now you have the modern-style FPS controls ;-) (after pressing the U)
Pretty much standard modern WASD except for USE key still being SPACE and JUMP is on the MMB instead. I can't put space for jump because I'm too used to it being the use key for old-school FPS games.

Why would you crouch with shift, or control? The pinky feels like it's gonna snap in two just after a few minutes.
I prefer crouching with c, or even z (default bind for every Build game)

Because with a WASD setup it frees up your other, more dominant fingers fingers for maneuvering. If your index finger is stuck on C for crouching rather than strafing you are trading one for the other, rather than having the ability to do both.
I decided to play it the way it was meant to be played, which is just using the cursor keys to aim. That said, I suck playing this way, and keep dying over and over again, so I might as well try some patch to get some decent mouse aiming.
