I played through the first 4 episodes a few years ago, and was sorta disappointed with the portraits in the fourth game, since they deviated a lot from the original style. Later on i found out that there had been an update and that the portraits had been changed to something a lot better. (in my own opinion)

Today i find myself replaying all the games with a friend on his mac, and as we boot up the 4th game i am shocked to find the old portraits lingering on my screen. I checked on my PC and here the new portraits work fine.

So what is the issue? Was the update never released for mac? Or is my download link messing something up?

It's not uncommon for patches not being applied to all platforms. Contact support and let them know.

For you as a workaround: The Mac version is the Windows version running in a wrapper to create a virtual Windows environment. You can check the contents of the wrapper by right-clicking (or Cmd-clicking) the app and selecting the Show Package Contents option. Inside you'll find the games Windows executable: Deception.exe. You can try replacing this executable with the one that comes with the Windows installation. They're indeed different. Install the game on a Windows PC, copy this Deception.exe over to your Mac and replace the exe inside the wrapper. Afterwards start the game on your Mac as usual.
