Letho's 1st encounter took me by surprise really...But the lifesaver for me was Quen sign II.
This on hard...made the fight a lot more easier.
Also once his shield is down give his a good or two shots with bombs or daggers lol.
I can guarantee you that I was lucky, I did not even wait until the cut scene was over for me to start clicking left and right, I may have had the Kraycan Armour on at that point or was it the Raven Armour from game 1 plus some upgrades to the sword and 1 or 2 potions active. Heck even I was surprised as ever when the next thing you know the cut scene came on! HA! I was even more surprised when it appeared that I had lost???

Now if you are all talking about the first fight with him at the Elven Garden, and right after we fell into the Pool area, I beat him in less then 10 seconds by just pounding at him with left-click and right-click and maybe one Igni and had him down to less then 1/4 health, then got a cut scene as if I had lost the battle, laying there in the corner and having him gloat about beating me. He soon went off and the game continued. And I was playing on Normal, so not sure if this is the battle all of you say is real tough or is there another battle (which I asume will be)? I also gcould have been very lucky?
Sometimes it is luck. Sometimes if I get the first attack down I can keep clicking LMB and keep the enemy stun-locked, but if one of the combat quirks pop up you give them an opportunity to fight back and sometimes turn you into a bouncy beach ball.

For example, I tried fighting slowly and noticed that on some occasions (15% maybe?) when you left-click Geralt either a) swings at thin air; b) run forward a bit and does not attack.

But fight with Letho is reasonable. I mean you have space to dodge and just one component.
I found the first enourter to be thought because letho is deadly in tight spaces
Use Aard to take down his shield. Don't try to stand toe to toe, he has big lumbering swings that are easy to dodge. Stick and move.

As soon as you start to anticipate his attacks, he does 1 of 3 things usually. Sword swipes, Fire or shield sign, Aard or bomb.

That's about it, so if you start to anticipate his next move, and dodge ahead of time, you'll have him miss alot more than not.

Yarden is kinda key too

I died like 30 times on him until I finally got it down. In the end, I beat him with full health and I'm sure I can do it again.

I had 2 points in Quen and 1 point in Aard. In short, you open with Aard to knock him back and then use Quen. Only attack him when his Quen is down.

While Letho's Quen is up, just run around him in circles until his shield is down. Open with Aard and repeat. Do this for about 15 minutes and you're solid.

Hope that helps.
I had no problem beating him after about 3 tries. Just roll around until his Quen is down, toss an Aard, two power slices, Aard, two power slices, and back up. He'll recast Quen, so just dance around until it is down again. Rinse, repeat.
I really enjoed this boss fight unlike the whole swamp thing where you have to press buttons (console crap). And unlike betsheda/ubisoft games letho gave me a fair challenge.

On hard, you jsut have to have like 3 or 4 bar of vigor.
Quen frequently to avoid bomb hit you, wait for his quen to wear off
The key is to throw bomb at him when his quen wear off so that he doesnt quen again

At the prologue i already was able to guess his whole personality.
