Enough with the child stuff
I understand that games of this nature have an inherent anti-social tinge to them but this particular game has not one but two very underage extensive scenes, so I cannot recommend it. Gameplay wise, it's pretty standard RPG fair, if you're coming from Sengoku Rance (Which would be rather great without the one underage single scene) it has much less depth and the story is less interesting. They haven't really improved their presentation in twenty years either, with very simplistic 3D graphics and static scene images. The added feature of Hero Syndrome makes grinding xp odd, you'll get hit with detriments but then get bonuses for curing it; however often you simply cannot cure the random diseases in progression yet so they just go away and you hope later you'll find the cure. It's not a particularly engaging feature, hard to utilize and more like "it just happens". With the premise that you're a doctor trying to figure out symptoms it would be engaging as a player to actually need to do something other than click a question mark button to magically be told what it needs. Maybe it gets more interesting later, when I ran into the second elementary school-underage scene I couldn't feel like I could ignore it anymore, since that character will be underage forever, and have more scenes. No thanks.
