Description
Pathologic 2 is a narrative-driven dramatic thriller about fighting a deadly outbreak in a secluded rural town. The town is dying. Face the realities of a collapsing society as you make difficult choices in seemingly lose-lose situations. The plague isn’t just a disease. You can’t save everyone.

The plague is devouring the town. The chief local healer is dead, and you are now to take his place. You’ll have to look for unexpected allies. The local kids are hiding something. Try playing by their rules.

You only have 12 days.


  • 12 days in an odd town ravaged by a deadly disease.
  • Time is of the essence: if you don’t manage it carefully, it’ll simply run out. You’ll have to choose how to spend the priceless minutes you have.
  • Survival thriller. You’ll have to manage your bodily functions, offsetting hunger, thirst, exhaustion, and so on. It doesn’t boil down to scavenging resources. Surviving on your own is hard; you’ll have to win over allies.
  • An uphill battle. Managing your bodily parameters may seem bearable at first, and as time goes by, it becomes harder and harder. Your own body is only waiting for an opportunity to give up and betray you. Things are changing from bad to worse and the odds are stacked against you.
  • A duel with an enemy you can’t kill. Your main foe is the plague itself, an incorporeal and malevolent entity that you have to defeat… without having the means to. It’s more powerful and more treacherous than you can imagine.
  • Loot, murder, mug, steal, barter, beg… or don’t. You need resources to survive, and it’s up to you how to obtain them.
  • The fights are short, ungraceful, and vicious. They’re not always lethal though. Many people—yourself included—would prefer to exchange their wallet for their life.

Unique and atmospheric

It's a shame it sold so badly, while run of the mill AAA trash rakes in billions. There's literally no other game like this one. It has the most interesting and eerie world I've ever seen, the story is gripping and the clock ticking mercilessly takes the whole experience to another level. You can't save everyone, you can't do everything you want, but you'll sure as hell be trying to. Only here after tending to a patient you end up having to cross an infected district with no immunity boosters left, and then breaking into people's homes to steal food, because you are starving and you spent the last of your money on antibiotics. Do yourself a favor. Play on the difficulty level labeled as "intended". It's brutal, but that's an important part of the story and only makes the game better. After finishing it, I feel like playing on easy would impact the experience very negatively.

No other game like it

This game deserves the highest praises possible. The cliche'd debate about if "video games are art" ends with this game, as solid proof that games can provoke deep thought, discussion, and stir the emotion. The gameplay itself can be written off as a "walking simulator" where the majority of the game is just moving around. But the grueling resource-intensive time-management system of sleep, food, thirst as you walk is definitely one of the low points of the game. But combined with the atmosphere and story, a case can be made that it enhances the experience. But after you get used to it, you really do feel a connection to the town and the situation you're in. I played through a bit of the original Pathologic, and this game is a vast improvement. You can feel the same energy and artistic vision as the original, which is pretty rough around the edges. But taking a game with such sheer potential as polishing it this much makes it into a true masterpiece. The music is part of what makes the atmosphere so powerful. Its enchanting and alien, creating an anguishing environment to set the stage. Its bleak, and gets the job done very well. I cant even describe the writing, its monumental. It challenges thought and makes you think with poetic dialogue. Much of it has many twist and turns, though not as many as in the original. It touches on the conflict of the old and new ways of life, existentialism, and constantly toys with you as you're struggling just to move on to the next task at hand. Overall, if you want to test your patience and experience an artistic and philosophical ride, buy this game as soon as you can. Try and play through in one go so the story and its messages stay with you as close as you can (Finished my first game throughout a single week). Its a unique game, and I don't think any other game will match this game anytime soon.

Incredibly Designed, Beautifully Written

Amazing game commenting on how the player is not expected to survive without sacrifice. Who or what you sacrifice is up to you. Interesting marriage of immersive-sim and survival-horror genres unlike anything else I have played. The story is incredibly interesting with deep, multi layered characters who all have their own say on matters with their own agendas. Be careful who you trust.

This game is unique

This game is hard to pin down, a story driven survival horror/thriller game that has some simulation elements. The worldbuilding and music are great. The gameplay and story meld togeather very well to make you invested in both, the game is really hard, but you improve yourself and your character troughout. It you are looking for a grim world to sink into look no further. The missionstructure is really addicting, and might keep you playing for hours at a time. Its so great that they made this game! i could not get into the first one because of its rough edges, but this one has been on my mind for a while. Good luck on your journey to uncover the mystery of Pathologic.

So unoptimized that I gave up

The game runs very slowly on my GF2070S, and this is a bummer. I enjoyed every bit of the very first Pathologic (that was released I believe in some mid-2000s), including story, written language (it's originally in Russian and very masterfully written), setting. This one, although promising at the beginning and bringing me back to the beloved and well-remembered setting, was unbearable to play on high or normal graphics settings. That's why I pass, at least until the devs look into the optimization.
