The Game Didn't Age That Well I Feel
I never played any of the metal gear games before so I approached this game without any nostalgia glasses on and was left disappointed. The graphics didn't feel like not pixel art, they felt bland (I've seen much better on the NES), the movement was stiff and not fluid at all, the bullets felt like tennis balls that disappear after traveling 10 meters and the punches felt like taser hits (except that the target didn't fall and the punches connected if sent in the relative vicinity of what I was aiming for). The stealth elements were ok. Planning my run through a level (which I felt were blandly designed, I threw the towel during level 2 out of sheer boredom) after a couple of trials and error was ok. Those mechanics were not jaw dropping in their implementation here, but would have been serviceable if integrated in a better (or more modern, if you prefer to look at it that way) game overall. This game might have been revolutionary back in the day, but I think it is important to rate this game with today's perspective in mind so that potential buyers can be aware of what they are getting. So to be clear, this is an old game that, I feel, did not age well. I think some NES games have aged well, just not this one (this is coming from someone who played a lot of NES games back in the day, but not this one and is looking at this game with unbiased eyes in 2021). If you played this back in the day or are a big fan of the series or are just someone who REALLY REALLY enjoys retro games (not just the timeless gems or retro-inspired indies, but also games that just feel old because they haven't aged that well), then you obviously don't need my opinion here. But for anyone else who is approaching this game without any pre-existing bias, I'd wager you won't get much enjoyment out of this title and might be better served by starting with a later installment of the franchise like Metal Gear Solid maybe.
