So, if you want to make multiple items of the same type, you have to select the alternate ingredients over and over. What should happen is the ingredients in the slots stay static after mixing, unless you run out of one.

It's especially annoying if there are multiple switches to make.
Aah, I see. Well, that should be patched. As soon (TM) as the official forums are up this should end up in a serious We Want Patch post.

Meanwhile, idea for workaround: Go to inventory, drop all ingredients you don't want to loose, enter meditation, do yo mojo, pick up items. I thing noone should steal them?
I have narrowed down the problem. I crafted other bombs and they appear in the inventory. My detective skills have lead me to the following conclusion:

Its not a bomb problem but a problem with whatever items I had in my pocket during the prologue. See during the prologue I equipped the freezing bombs, rusty throwing daggers and a conflagration trap in my pockets to use. After the prologue at the start of chapter one I had lost all these items, they where neither in pocket or inventory. So what I believe is happening is that these three items are in an invisible pocket that I cannot use, and when I create more the game stacks them in this invisible pocket.

Any suggestions on how to make this known to CD projekt so they can fix it?

In the meantime I suggest to anyone playing the game form the beginning to un-equip anything they put in their pockets before the end.

May put the game on hold for a bit till some patches arrive, not too keen on being a games tester in my spare time.
I just recently ran into the same problem as you Guillerms, but it didn't manifest until act 3. After using up the last of my fire bombs(dancing star or something) I tried making new ones; I get feedback from the alchemy interface that my bombs are made but they don't turn up in my inventory(neither in the bomb tab or the all tab). Worst part is that the bombs still carry weight! I just booted the game to test if the bug was still there, and after I crafted 10 dancing stars my total weight increased by 5 and my bombs were still nowhere to be found. Pretty big bug I'd say.
There appears to be some kind of bug with items in the pockets. If you run out of the original number of items that you've pocketed, then no matter how many you've picked or crafted afterwards, you can't use the item before you move it from your pockets back to your inventory. After that you can pocket it again and it'll work properly.
Do the Fight Club quest and you'll have them back - I had this problem as well!!
