Armor: Vran Armor (for swordsmanship heavy builds)/ Ysgith armor (for magic heavy builds)
Steel Sword: Forgotten Vran Sword (or Caerme - a close second, for people who chose a certain path and is unable to get the Vran Sword)

Although on my mage heavy build I liked the Operator's staff better than the forgotten Vran sword. Operators staff has 40-60 damage I, believe and a mighty 80% incineration plus 10% freeze (if memory serves me right).
In the end I had 8 vigor points, 0.5 vigor regen in and out of combat and 80-97 damage with potions. The best thing was, if you only use Greater Critical Effects mutagens and boost sign damage and intensity as far as possible you get exploding frozen bodies left and right and you can 1-2 shot a group of harpies from your quen damage plus a single igni area spell alone. Actually from the Operator's staff they dance around drunken from being set on fire, then the next or the one after the next hit freezes them and makes them explode :).
My mage build is so much fun, I load it up from time to time just to replay the last battles in act 3. (on normal difficulty).
Hoping for some decent dlc or expansion pack so I can use it to full effect again :)
The sword with 25% added bonus against large creatures and gargoyles is called "Deithwen" and is very useful against a certain large creature at the end.
