So below is what i've learned from playing the game about the endings, the world and Yennifer. Some thoughts and some guesswork.
Many spoilers of both games and the witchers world so go away if you dont want to read.
Just felt i shouldget it written down to see if i have things right and see what others came up with.

Nilfgaard sent Shilard to the Northern Kings to offer them some deal. We dont know what the deal is, but according to Shilards 3rd letter to the Nilfgaardian Emporer we know King Henselt refused.
Nilfgaard also hired Letho and the witcher school of vipers to assassinate the Northern Kings, and in the process set-up the downfall of the Lodge of Sorceressess. This sets up chaos in the North to help the incoming Nilfgaardian invasion.
Shilard, when he and King Radovid are in Philippas prison claim that Triss gave a list of the Lodges members and seems to have convinced the King that the Lodge are behind all assassinations.
The Nilfgaardian Matsen in their camp claim that Triss confessed about the Lodges actions after showing her confessions from other Lodge members.
Triss when saved claims they got no info from her.
The Lodge wants its own state to rule, and has chosen the Pontar Valley. They, or atleast Philippa and Sile hired Letho and setup Demevend's assassination and then backed and mind -controlled Saskia to further achieve their goal.
Philippa claims Triss is innocent in regards to the assassinations, but hasn't been honest to Geralt in regards to many other matters.
According to Triss, the Lodge wasn't keen on her close relationship with Geralt and manipulated Triss. Through her Geralt has been given select information and depending on circumstances twisted facts. Also Sile fears Geralt.
Triss is/was a member of the Lodge but due to her close relationship with Geralt, hasn't been fully trusted and when questioning the Lodges plans on how they were going to overthrow Demevend, wasn't invited to anymore gatherings. She had suspicions after Demevends death but no evidence.
Triss says she is a member of the Lodge along with 8 other sorceressess and Philippa and Sile practically run in themselves.
If we save Triss, the new Conclave/Council of Sorcerors is created with the chief interest being the well-being and prosperity of the Northern Kingdoms, but 1 or more Kingdom may be divided or be leaderless.
If we do not go to save Triss then Shilard will incite chaos in the negotiations, the Council/Conclave of Sorcerers wont be created and lots of mages being killed by the various factions, but we can help stablize a Northern Kingdom.
Letho says Yennifer is in Nilfgaard (with the Emporer?). Also noted in one of Shilards letters to the Emporer he addresses the Emporers Consort. Possibly memory loss Yennifer (Wild speculation i know).
Sile also knows that Yennifer is in Nilfgaard if you save her from popping. I'm guessing the whole Lodge including Triss know, and thats the main secret Triss is hiding from Geralt as Philippa claims Triss hasnt been totally honest with him. Fits with TW1's mirror scene too.
The Wild Hunt moves from North to South every few years and are the spectral avatars of powerful Elven riders from another dimension(A Sphere in the witcher universe I believe ) searching for something to open up the gates between worlds forever. They can cross over but its not easy so they send their spectral form, unless its a special mission and then they come fully over.
(after reading the witcher wikia a bit, it seems they are after Sources. Specifically Powerful magic users with Elven blood. Like Alvin from TW1, and it seems Ciri, Geralt and Yennifers adopted daugter who left the world, and is also apparently part of a prophecy involving the Nilfgaardian Emporer, giving him a reason to be keeping Yennifer as she is a link to Ciri. And i'm guessing the Wild Hunt let Geralt go to see if he would lead them to Ciri also.)
