When I try to change gears, it usually takes 2-3 button presses before it works. Is this a problem with my game or am I doing something wrong?

I have the same problem with automatic transmission, when I want to use the reverse gear. Takes a lot of button presses.
That's interesting. I don't use manual transmission, but I have noticed something similar when trying to navigate the in-game menus with the keyboard (arrow keys).

Does that seem like the same issue?

I think the menu navigation keys are fixed, but out of interest are you using the arrow keys for gear changes? If so, does the problem occur if you bind those commands to different keys?

Nope, I use the default keys for changing gears (S and A if I remember correctly). I use mouse in the menus, so I don't know if it's the same problem.

Sorry guys for necroing this years old thread, but i just wanted to inform you that the transmission problem is directly tied to the framerate of the game. The higher the framerate the worse it gets. Either lower you CPU clockspeed while playing or cap the framerate. Anything above 60 is not advised. Cheers
