Well, I managed to get the game to run in Glide mode.
I think the problem was RadeonPro.
I added the Screamer4x4_glide.exe to the compatibility list and deactivated the API-detection. Since then, the game runs flawlessly.
I also installed the latest nGlide-version before that, but I don't think this is necessary.
Thank you! This was exactly my problem. I tried all of the recommendations from GoG with no luck. I'm using a Logitech mouse and have SetPoint installed. Close SetPoint resolve my problems.

I'm running Windows 8.1 with a NVidia 660M, i7.

brunchero: I had the same crash-freeze problems and could resolve them on my windows 7 x64 with Radeon HD5770 (DX11), Core2Duo E8400.

Short story:
It was the Logitech SetPoint Process, when I kill "SetPoint" the game starts properly.

I used the GL-version, which gave my the best results. Neither admin privileges nor any compatibility settings were necessary to play. (I use an unprivileged user with active UAC.)

Long story:
I used the admin user and msconfig as suggested in the troubleshooting tips* to perform a "clean reboot". Please BE CAREFUL there and know what you do. With startup items disabled, but all services still enabled, I was able to play the game already. After enabling the processes haphazardly and some reboots I finally found the SetPoint process being the showstopper. I went back to normal reboot and simply killed the process before launching the game, which was successful.

For launch tests make sure the task manager is closed, otherwise it will be frozen like the explorer and other opened applications. Launch it with ctrl-alt-del after a game crash and kill the game executable and the "explorer" process, then on tab "applications" click "new task" and start a new "explorer". That will fix the frozen desktop manager without a reboot.

i have the same issue with this game. the issue is clearly something in the game, because i dont use logitech gear, and i dont have a radeon card. so while adjusting those programs might help, there's clearly something broken here that gog needs to fix

Sorry for the late answer, didn't see your reply until now.
Rightclick on the RadeonPro-icon, select Settings, then select Compatibilitylist (or similar, mine is in German).
Click the Add-button and in the opened file-browser-window, search for the "Screamer4x4_glide.exe" (in the folder where you installed your game).
Then you can select the check-box for API-Detection on the right side (don't forget to save your settings at the end :) ).

I've got the same problem. I can't launch Screamer 4x4. Neither glide, D3D or OpenGL. It always ends with being unable to do anything, since every click results in a "ding". Neither any compatibility mode or launching with admin rights makes any differnce. There isn't any logitech software runing, so I can rule that one out. I'm using win7 64bit with 16gb RAM and a Radeon 290x.

If anyone would have any more ideas, I would be very grateful!
I'm getting the same thing, except I've got Windows 10 with an nvidia card. The thing is, it worked with this same hardware when I was running Windows 8.1, but not since the upgrade. It just crashes when it should be showing the intro video.

Same issue here, unfortunately.

EDIT ------
Partially solved this by going to the game folder and running nglide_config.exe to set my resolution, then running Screamer4x4_glide.exe to play.

The game crashes often and I have to sign out/ sign in again to restore Windows in working order after each crash.

If these solutions are not working, try to close nvcontainer.exe in the task manager.
