If you're interested:

Open the device manager, find the graphics card and check its properties, in the Details tab there's a menu where you can select "Hardware Ids". Now you should see one or more lines containing VEN_<something>&DEV_<something else>. That's short for vendor and device and is the unique ID for your card, using that and a Google search (or us, though we'll just search Google for you because we don't actually have this stuff memorised) you can find out what it actually is in human language.

For example, my Radeon R9 270X is VEN_1002 (1002 is AMD's vendor/manufacturer ID, separating it from other manufacturers, like, say, Intel which has "8086" as their ID) and DEV_6810 (6810 the chip's ID separating it from every other AMD product, like, say, the Radeon 5750 which is known as "68be", or the HDMI audio device present on either card, which I'm not bothering to name).

Thank you very much! So what about running W3? Damn, I can run BF4 on ultra! The only game i couldnt play was AC unity. I dunno if it is because of my computer or because the game was a piece of crap.
If you are unsure about your hardware you can always use a system information tool like Aida64, SIW or Speccy. While the first two give you more in-depth information and also have additional features like benchmarking or remote control via tablet, Speccy is sufficient and probably better suited for beginners. And it is free for home users as well.

Well, they do say that it requires an HD 7870 at minimum, and while the 6950 is a generation older (a 6* versus a 7*), it's also a level higher (a *9* versus an *8*). According to GpuBoss there isn't much of a difference, and I would wager that the game will run... but probably not very fast, and you'll likely need to shove most graphical settings down to the bottom or nearly there.

Whether or not it'll be playable mostly depends on your own requirements (to you consider a game to be unplayable if it dips below 30 FPS every once in a while? how about if it never reaches above that? do you require 1920x1080, or can you make do with 1280x720? 1024x576?). I don't know how well the game will run at minimum requirements (only GOG knows how well the current build runs), but I would guess that you'll likely have to settle at the lower end of performance and graphical fidelity.

There is also the possibility that the game could use graphic features not available in the 6000 series. You are certainly on the borderline.
