To start off with:
- I tried running it in Windowed mode.
- I tried running it in 1600x900 resolution
- I tried changing compatbility to Windows XP Service Pack 3
- I even tried dgVoodoo, but that didn't work.

Any suggestions? I had the same problem with the Steam version of a different TR game (Chronicles), and the GOG version of Chronicles just hangs on a black screen after I try to start it up. I downloaded the GOG versions hoping to have a different experience, but that is not the case.

Did you try the multipatch ?
I have the same problem on Windows 10 even with the muli-patch.
This is odd, because on my other PC which runs Windows 11, it runs straight out the box on GOG without any patches.
I'll keep digging.
I had the same issue and tried a number of different things, eventually narrowing it down to a resolution issue. Even with the widescreen patch, this game seems to have issues with some widescreen resolutions. The solution for me was to try them all, and eventually I found one that works with my monitor. If all else fails, you could just use one of the 4:3 resolutions.
