You may or may not be aware of these but they are pretty big. I was unaware of these bugs in previous versions so its possible they could have been around and I only noticed them until now.
1. When you fast-travel, meditate, load a saved game, or after some cutscenes, the bonus damage from red mutagens isn't properly calculated towards your overall sword damage unless you remove them and then place them back into the mutagen slots. You don't need to refresh the skills, just the mutagens.
-update- I guess this issue affects all mutagen types, and does not calculate the bonuses properly unless you refresh them.
2. Focus ability is bugged. Its the one ability in the entire game that actually works when its not placed into one of the skill slots. If you place it into a skill slot, depending on how many adrenaline points you generate in combat it actually lowers damage instead of increasing it. If you do not place Focus in a skill slot, then gaining adrenaline points gives you more damage. (based on the character-stats page)
This is based on the numbers that I'm looking at when I changed stuff around and looked at the character stats page. So either this stuff is bugged, or the character stats that is being displayed is bugged and wrong.
Either way, one thing or the other is bugged.
