If you're experiencing these issue since today's DLC update, please use this temporary workaround.
You should move the following file outside of the binx64 folder:
d3d11.dll

The default installation folder for The Witcher 3 is C:GOG GamesThe Witcher 3 Wild Hunt

If this doesn't help, please try moving these to a different folder as well

Well I tried

Moving d3d11.dll did not help anything.

About the filelist I found only these and moved them:
d3d11.dll
dinput8.dll
dxgi.dll
GogGalaxyHooks.dll
xinput1_3.dll

Now it won't even try to start when clicking Play.

Game was working without problems before this day.
I have this problem and this temporary solution does not work for me. This problem has started when i have updated GOG client.

funny thing is I have no problems, but my gf who runs almost the same kinda system as me can't start the game. This is pretty annoying, fix your stuff guys.... plz.....
Thanks to mmarci to leading me here, removing d3d11 from the folder solved it... but clicking "Disable API Monitoring" in the RadeonPro options did that as well.

Thanks Firek for the temp workaround. Removing the dinput8.dll, dxgi.dll, GogGalaxyHooks.ddl and xinput1_3.dll lets my Mirillis Action working again perfect in directx/opengl mode. First I´ve figured out another workaround for me by changing from directx/opengl mode to desktop mode in the action menu, but some staff of action wasn´t working as it should be with my wo. With your wo all is fine again.

I haven´t removed the d3d11.dll, because I use a special one from a mod, which works together with action and not the original one.

Thanks for this. It fixed the Steam Overlay for me. I wasn't able to take screenshots anymore nor use the FPS counter. Moving or renaming d3d11.dll breaks the SweetFX preset I'm using so that wasn't an option for me.

I've been launching Galaxy from Steam, then launching the game from Galaxy, so that I could use the Steam overlay to take screenshots. As of the latest DLC, the game was crashing to desktop as soon as I launched it.

The work-around did fix the problem I was having. In order to get it sorted out, I had to move all of the .dll files that were added to the bin\X64 directory when the DLC was installed:
- d3d11.dll,
- dinput8.dll,
- dxgi.dll,
- GogGalaxyHooks.dll,
- xinput1_3.dll, and
- xinput1_3wrapper.dll.

The game launched without crashing after I moved just d3d11.dll, but the overlay didn't work until I moved all of the other DLL files listed above.
Finally!!! It works for me renaming d3d11.dll and dxgi.dll (I think last time i tried i did wrong). OBS works now and i can continue recording and streaming.
Tried deleting only dxgi.dll and d3d11.dll from the folder and this worked for me!
Done all of this and its still not working, been messing about with this game all day trying to get it to work. shouldnt have to do this when you buy a game.
all I had to do was move d3d11.dll out of the x64 folder and Rivatuner worked again with no adverse reactions in game.
