I have an issue with newly acquired achievements. It says the last one I got was 20 days ago before patch 4.03, which is false because a small icon still pops up during gameplay. When I check an activity section it says "50 hours played" 7 days ago. Since then, there has been no more information about new achievements.

I tried to manage my installation by verifying/repairing game files but it did not help. I also removed GOG GALAXY from my PC and installed it again. Nothing has changed though. Achievements are stuck at 14% even though I got at least 10 more pop-up icons while playing.

Do you have any idea what may cause such an issue? Or have you experienced something similar before?

It's most likely your achievements are unlocked in base version of the game, instead of complete edition.
I had the same issue, rolling back to 4.02 fixed this (although all achievements you have unlocked since installing 4.03 have to be unlocked again).

Seems like it doesn't work on the GOG GALAXY app. When I open a website with achievements it works normally.

there's no difference. on gog galaxy website it shows "The Witcher 3: Wild Hunt" (not Complete Edition version) achievements, if you play on 4.03 patch. the only way to continue performing achievements is to rollback to 4.02/4.02 hotfix version.
