In chapter 2 I can't seem to find the entrace to the Catacombs where the Dun Banner is hidden. Do I have to cross the fog again to get it to or not? I am currently in the Dwarf city.
No, in fact those catacombs are like almost the opposite direction from the battlefield in that forest out the other gate.

i have the other 2 items, and im trying to get the dun banner. i'm at the quest marker, I enter the crypt.. and the quest marker still points at the front door of the crypt. so when im outside, it tells me to go in and when i'm inside it tells me to go out.

i've searched the smal crypt, killed all the wraiths.. what the hell do I do now
You mentioned a crypt where you killed wraiths...
Did you use Aard to destroy walls in that crypt? In the correct crypt there are maybe ~6 walls that can be destroyed and the quest marker is behind one of those walls.

hey please can someone tell me where I can find the rose of remembrance in this chapter 2 I gave it to Triss in chapter 1
hrm, nothing to do with dun banner in that location .. and it turns outnI already have the dun banner cloak in my inventory

so I have all 3 items im supposed to have.. but the game is telling me i only have 2/3

is the dun banner cloak not what I need for that quest? it's a quest item so.. it must be, surely

No the dun banner cloak isn't the same as the dun banner itself. It's marked as a quest item but I haven't found a use for it.
However, if you have the cloak you should have the banner.

The three items you need are the banner from the crypt, the sword from Saskia and the spear from the dwarven poker player. Which two have you got?
i have all 3 now, I hadn't noticed the last wall where the main wraith was hiding

the dun banner cloak was so that the wraith woulkd believe you were a member of the dun banner, even if you got a question wrong.. I think
Can anyone please advice, where to start with this quest? Where is the Dun Banner standard and the Vanegrifts sword? I was hoping that after those dwarves told me where to go at the table, I will at least get a quest mark, yet nothing. It really starts to annoy me having to run around like idiot with nearly every quest :-(

What does your quest say, because I assume you're on the Roche path, and the only two times where they didn't give you clear cut guidance, as far as I remember, were when the quests essentially told you to wait for an opportunity to cross the battlefield or wait for the story to open up, which means you go do other things until they become active again.

No I am on Scoiatel path. The quest says find Dun Banner's standard and Vandergrift's sword. Then go talk to Philipa about other two items. No location mark, nothing. And as I said, I am tired of running around. So anyone can please advice where the hell are these two items?

That's the general guideline. For the Iorveth side, you should have also been told to speak to the local people to gather information. Upon speaking to certain individuals, they will direct you to quest specific help regarding those items.

In that particular case, you're supposed to be "running blind" as you put it. That said, I imagine the game guide that came with the game has the answers you're looking for if you don't want to bother. ;p
