Feel free to take a look at this list and provide any additional information. If you have experienced these bugs yourself please confirm, if you know of fixes please tell us. Thanks!

1) Geralt will randomly sheath his weapon and refuse to take it out. This occurs at random times.

2) Children are very hard to select for conversation. You can only speak to children when running up to them from a certain angle, making the task of clicking them nearly impossible.

3) Red dots on the mini map remain after enemies are killed.

4) Sometimes it becomes impossible to pick up loot, no matter what angle you approach. This occurs at random times and to random loot.

5) Items become highlighted by using the Witcher's medallion, but remain unsearchable. This does not include items out of reach; this has to do with items that are right next to Geralt.

6) When using the Swordmanship group finisher, live actions still take place. For instance, while Geralt is shown killing three enemies, he is simultaneously taking damage from other enemies that are not shown. Thus, when the finisher cinematic is done playing, Geralt dies.

7) During the Kayran boss fight Geralt will not attack the tentacles for no apparent reason. Right and Left mouse clicks do not register an attack. This occurs randomly.

8) The book Temerian Royal Dynasty claims Geralt killed Adda when trying to remove the curse from her. However, this is incorrect if a player loads a Witcher 1 save, where Adda is freed from the curse without dying.

9) Flares will bounce through walls.

10) When fighting in the mist (during the entrance into Chapter 2) stunning and killing the draugir can result in Geralt’s death. The problem is the cut scene finisher takes place outside of the protective bubble. Thus, Geralt takes damage while killing the draugir.

11) Gynvael Aedd is supposed to increase damage done to all necrophages by 1004%. The weapon does not increase damage done to nechrophages.

12) Cut scene kill sequences will sometimes take place indoors when Geralt is outdoors.

13) Melitele’s Heart Quest: When protecting Anezka if the wraiths get to close to her she will stop casting her spell and wraiths will spawn infinitely.

14) After the love scene in the elven ruins Geralt will have all of his gear unequipped.

15) Don’t know if this is a bug, but Newboy’s Medallion only seems to grant Geralt +10 Vitality outside of combat. Maybe that is working as intended though.

16) Used trap seems to register as being “taller” than they appear. Throwing a dagger over a location where a used trap is will stops your dagger from reaching its intended destination.

17) More of a suggestion that an bug: Subterranean Life Quest: The dwarves that accompany you into the mine could stand to be more responsive in getting out of Geralt’s way. Being charged by a Bullvore and having a wall of dwarves block your way is never fun.

18) The Talent Catalysis which on the first rank grants: Effects of all potions taken +15% Negative effects of all potions taken -30% seems to not work properly. Many potions including Rook, Thunderbolt etc, all seem to be incorrectly affected by this talent. Unless the math for this game is done in a way I do not understand. The same is true for rank 2.

19) The quest Lost Lambs cannot be completed because Zyvik disappears from the camp. Also, if you track the quest to find Zyvik, the marker on the map is on the other side of fog.

20) The very first flashback scene in the game seems to have sound problems. The sound is very choppy, but the video will continue to play.

This happened to me after a normal stun kill

Akhotnik: 1) Geralt will randomly sheath his weapon and refuse to take it out. This occurs at random times.
I've never had him automatically sheathe his weapon, and then refuse to take it out, but it's a fairly known bug by now that key input sometimes stop working (either partially, or altogether). It's possible that it's related to what you describe, so all we can do is hope they're working on it for 1.3.
avatar
Akhotnik: 2) Children are very hard to select for conversation. You can only speak to children when running up to them from a certain angle, making the task of clicking them nearly impossible.
Interacting with children has never been a problem for me, so I don't know exactly what this might be. They're as easy to select and talk to as any other NPC. I know there's a bug with older versions of AMD drivers (correct me if I'm wrong, please), causing various interactions in the environments to be wonky or non-functional in general. If your drivers are up to date, it's obviously not that, though.

Akhotnik: 3) Red dots on the mini map remain after enemies are killed.
Aye, I've noticed this as well. A very minor nuisance that I believe Dragon Age 2 also suffered from (not the same engine, so it doesn't matter). It doesn't happen all the time, however.

Akhotnik: 4) Sometimes it becomes impossible to pick up loot, no matter what angle you approach. This occurs at random times and to random loot.

Akhotnik: 5) Items become highlighted by using the Witcher's medallion, but remain unsearchable. This does not include items out of reach; this has to do with items that are right next to Geralt.
Yes, some containers are either very difficult (or outright impossible) to loot, due to their placement. Even if they are 'within reach' you might have to approach them from a very specific angle in order to loot them - there are several found in the game that I've never been able to reach even then. Again, a minor nuisance, since I've never seen this happen to any important containers; only those containing random miscellanea. If you're talking about loot containers dropped by enemies, however, we might be speaking of a similar issue to what you describe in point 2, because I've never had trouble looting enemy drops. Sometimes they get stuck within the geometry, such as under a rock, however, and thus become a bit trickier to reach.

Akhotnik: 6) When using the Swordmanship group finisher, live actions still take place. For instance, while Geralt is shown killing three enemies, he is simultaneously taking damage from other enemies that are not shown. Thus, when the finisher cinematic is done playing, Geralt dies.
Hmm... As far as I know, you're not supposed to be able to take damage even from the group finisher, so there's clearly something wrong there. Using any finisher (group triggered by adrenaline move, or the standard one when the enemy is stunned or knocked down), does not stop time, but all the enemies are able to do is move around. They are not supposed to be able to attack.
avatar
Akhotnik: 7) During the Kayran boss fight Geralt will not attack the tentacles for no apparent reason. Right and Left mouse clicks do not register an attack. This occurs randomly.
Never had this happen to me, specifically. It sounds like it can be related to what you bring up in point 1, however - where the controls simply stop responding. Does this occur even after you've trapped the tentacle? It's possible attacks won't register unless you're actually able to hit it; i.e. it must be trapped first.
avatar
Akhotnik: 8) The book Temerian Royal Dynasty claims Geralt killed Adda when trying to remove the curse from her. However, this is incorrect if a player loads a Witcher 1 save, where Adda is freed from the curse without dying.
Yes, the flavour text you see when you manually read the book from the inventory, is faulty. If you check the actual journal entry you get from reading the book, however, it will display the correct state of your imported save, so it's just an error in the writing of the actual book item, and you can rest easy knowing that she's still happily alive and married. ;)
avatar
Akhotnik: 9) Flares will bounce through walls.
I have not used flares more than a couple of times, but I seem to recall noticing this bug, as well. If not every time I hit a wall, but at least sometimes.
avatar
Akhotnik: 10) When fighting in the mist (during the entrance into Chapter 2) stunning and killing the draugir can result in Geralt’s death. The problem is the cut scene finisher takes place outside of the protective bubble. Thus, Geralt takes damage while killing the draugir.
I can confirm this - it has led to many deaths during playthroughs on higher difficulty levels. I simply stopped using finishers during the escort scene (I even made draugirs completely immune to stun and knockdown). Definitely a bug within the script detailing where to 'teleport' Geralt and the enemy, when executing a finisher.
avatar
Akhotnik: 11) Gynvael Aedd is supposed to increase damage done to all necrophages by 1004%. The weapon does not increase damage done to nechrophages.
Yes, there's an error in the file responsible for the sword's stats. I don't know what value they intended to add, but I assume it was 10%. Filling in the numbers only a bit wrong, might cause errors like this. I can easily fix it for you in just a minute or two, and send over a fixed file if you want to use the sword very badly. (I'm going to shamelessly advertise for my own mod, here; it re-balances and changes every item in the game for a more even challenge rating and more interesting items overall.)
avatar
Akhotnik: 12) Cut scene kill sequences will sometimes take place indoors when Geralt is outdoors.
Yep, I can confirm this as well. Many people have brought this up - the mechanics for how the finisher cutscenes are handled, are very buggy overall (adding the other problems you've already brought up here), and people have been asking for both fixes, and the option to disable these cutscenes altogether.
avatar
Akhotnik: 13) Melitele’s Heart Quest: When protecting Anezka if the wraiths get to close to her she will stop casting her spell and wraiths will spawn infinitely.
Yes, another confirmed bug. It usually only happens if you're standing close to her when the battle begins, and you draw your sword, causing her to enter her 'scared innocent stance.' What I do is to immediately run down the little slope once I've triggered the event to start, thus staying away from her when the wraiths finally arrive, and then try not to run too close to her while I'm fighting.
avatar
Akhotnik: 14) After the love scene in the elven ruins Geralt will have all of his gear unequipped.
Yes, but not always (at least not for me). It usually happens when I'm skipping through the cutscene/dialogue very fast - it's like the game doesn't have time to execute all the scripts or something. Other oddities can be caused by doing this, as well, that I've noticed.
avatar
Akhotnik: 15) Don’t know if this is a bug, but Newboy’s Medallion only seems to grant Geralt +10 Vitality outside of combat. Maybe that is working as intended though.
Yes, there are two values for vitality (and vigor) regeneration: One for out of combat, and one while you're in combat. The former doesn't specify which it is in the tooltip, but the latter does say 'while in combat.' So yes, it's working as intended.
avatar
Akhotnik: 16) Used trap seems to register as being “taller” than they appear. Throwing a dagger over a location where a used trap is will stops your dagger from reaching its intended destination.
I've noticed this as well (both used and those yet to be triggered, as well, I think). They act as an impassable obstacle, meaning you can't move across them either, and yes, they seem to be taller than the texture model shows.
avatar
Akhotnik: 17) More of a suggestion that an bug: Subterranean Life Quest: The dwarves that accompany you into the mine could stand to be more responsive in getting out of Geralt’s way. Being charged by a Bullvore and having a wall of dwarves block your way is never fun.
Yeah, the escort mechanics can definitely be improved. It's an awesome quest, and I love those dwarves, but they follow you way too closely. They should be programmed to keep more of a distance from Geralt while moving around, and to move out of the way immediately if Geralt bumps into them. I would also like to have the 'attack area range' increase; as it is now, I can take only a few steps away from them before they stop attacking an engaged enemy, and instead start running towards me.

[For crying out loud... 'Your post is too long.' I can't add the rest until someone else writes a reply first.]

Akhotnik: ...
It'd probably be best if you added your info to this thread.

1) I agree. Can't exactly reproduce it at will though.

2) No idea. But children don't have anything to offer in terms of gameplay.

3) Yup.

4) I'm thinking that the targetting enemies and loot-picking use the same mechanism, agree that it's far from optimal. To pick up loots, try walking instead of running.

5) Yup. I think I saw this in many threads where items are highlighted but not lootable.

6) No idea about this. My impression when using the finisher on stunned opponents is that while the cinematics is playing, opponents can move but not attack you. Not sure about group finishers.

7) Having fought the Kayran something like 30-40 times, I haven't seen this issue. I know an issue where pressing attack causes Geralt to swing at thin air, or move forward + sidestep without attacking though.

8) I agree. I haven't played with an imported save, but I imagine the content of the book is not altered even if Adda was alive.

9) No idea about this.

10) Yup. It has been discussed in several threads.

11) Never bought this sword before even though I've seen it a couple of times. Did you test it with rotfiends? I think they are the only necrophages that appears at that stage.

12) I agree. In general, cutscene finishers shifts your location temporarily, hence some (lie me) has proposed to make the cutscenes optional.

13) Never encountered this myself, but I have seen several threads with this. It might be that she never started casting at all because you draw your sword too early or something. She is oblivious to the wraith's presence, nor do the wraiths attack her, if everything is working correctly.

14) Haven't check lol. But I guess you do take everything off when you have sex??

15) Not sure if it grants +10 vitality, but it specified that it adds 1 vitality regeneration out of combat, and yes, it's not very useful.

16) Hmm.... no idea about this.

17) In general, everyone follows you too closely (not to be confused with when you are supposed to follow them) which is an enormous pain in the ass.

18) Yup. See this for the actual effects of the skill. In addition to this, the bonuses specified in Specialization: Oil and Impregnation (see [url=http://www.gog.com/en/forum/the_witcher_2/the_nearly_complete_mutagen_guide]here) is also incorrect.

19) Not sure about other side of the fog. But I've seen in another thread where the OP couldn't find Zyvik.

20) No idea about this.

Can't you just choose "new post" at the bottom right corner if your current post is too long??

Many percentage stats like these, shave off a percentage of the value, and not a raw percentage decrease. For example: -10% negative effects on a potion that has -60% vitalty, by default, won't decrease the value to -50%, but rather by 10% of 60% - which in that case will be -6%, resulting in -54% vitality when drinking that potion, and having that skill. I'm having trouble explaining it clearly, but maybe you see my point. That being said, I don't know enough about potions to say anything for sure. Could you possible give a bit more detail on the matter, as to why you suspect the skill isn't applying the numbers correctly? A concrete example could help.

Oh my... That's never happened to me. The quest markers can sometimes be screwy (something they'll fix in 1.3, I read), but Zyvik should always be wandering around the camp, often sitting down outside the quartermaster's tent. While typing this, it does sound a bit familiar, though. It's possible someone else has mentioned this on the forums, before.

Akhotnik: 20) The very first flashback scene in the game seems to have sound problems. The sound is very choppy, but the video will continue to play.
Nothing I've ever heard anyone else mention, and this has also never happened to me. Make sure the graphics and sound drivers are up to date; other than that, I have no further advice to give. Sorry.

vAddicatedGamer: Can't you just choose "new post" at the bottom right corner if your current post is too long??
Yes, but doing so when you're the author of the most recent post already, only makes you add to your previous post. I was able to squeeze in a bit more this way, but it still showed up as too long for the last few. :(

Thank you guys for looking over the bugs. Thanks for the mod Kindo.

I'll go ahead and transfer the list over to the thread you mention vAddictedGamer.

Item 5 (items highlighted but not lootable) is not always a bug.

Sometimes an item is highlighted, but Geralt cannot get close enough to loot it. That's a bug.

Other times, an item is highlighted, but it is not intended to be lootable. Clutter items that fall noisily and attract unwanted attention, for example. This is by design and not a bug.

20) The very first flashback scene in the game seems to have sound problems. The sound is very choppy, but the video will continue to play.
Ran into this myself -- that flashback, and /only/ that flashback (went through both Act IIs, as well).

HW - recent Thinkpad W520, Intel 2720QM, Quadro 2000M, 8GB RAM, untweaked drivers. Rest of the game ran smoothly.
lw2jgog do you play in 5.1 surround sound or higher? I have a feeling all people who have had this issue may have that in common.

Akhotnik: lw2jgog do you play in 5.1 surround sound or higher? I have a feeling all people who have had this issue may have that in common.
Doubt it. I'm relying on the built-in audio of a non-gamer laptop. :D
Ah okay, was wondering if you were using a head set with surround sound. I know someone using the Logitech G-15 with built in 7.1 having the same issue as us.
