It is installed when you install Black Plague (since it's an expansion rather than a stand-alone game). The shortcut is in the start menu.

Um, what does it add? I just finished playing through Black Plague for the first time, so I wouldn't be able to notice any differences.

It doesn't add anything to the Black Plague campaign. It's it's own campaign.

I think the only reason it was released as an expansion was because it was so short.
