I have Rebirth, Afterbirth and Afterbirth+ at Steam, and played it as start point. With no question...
In GOG I have only Rebirth.
The question is:
If I finish it without any DLC, and then purchase them - can I play thru my original save file, or I must start from scratch?

You can continue a save across DLCs and they are saved in different folders so you can uninstall DLCs to go back to a previous one if you wanted. This is what I'm currently doing, playing the base game first and installing and playing the DLCs in order.
