Seriously, every time I go in there, I either lose a bunch of officers, or hostages die. What the heck am I doing wrong? I've tried every weapon, every entry point, it's just getting ridiculous!
AT start, pick first door up the stairs, and then enter the skywalk, at the end of the skywalk put a charge on the door and drop a CS gas on the floor, and come back to the entrance, then blow it. Slowly work your way to the secondary entrance using blue on left and red on right to provide cover. once you know the left storage area is clear, clear the bar area, and work your way back around to the hall way.

After clearing normaly around the dressing rooms and upstairs control rooms, work your way back to the main studio part.

Before entering the "studio part" of the DBN studio mission, try using CS gas on the upper draft sound reducing panels to gas the studio before entering.

I got past it, finally.

I just...it's really irritating when ONE GUY takes out all my guys including ME, or when one of them dies while he's in the middle of throwing a flash grenade, or even mirroring for suspects...I don't know...the game play is like, inconsistent at times and I think mainly it's due to the AI.

That's one of my favourites tricks with this mission. There's a small gap at the top of those barriers behind the stage, and if you aim up at the lower edge of that and use a full-strength throw, you'll pop the CS through the gap.
